To streamline the referral process, we offer a referral form for dentists and dental offices. Submitting a referral allows us to promptly contact the patient, coordinate treatment planning, and ensure continuity of care.
How to Refer a Patient
Please complete the referral form and include any relevant clinical information, such as:
- Reason for referral
- Proposed extractions or completed dental treatment
- Radiographs (PAN etc.)
- Medical considerations relevant to denture treatment
Once the referral is received, our team will:
- Contact the patient to schedule a consultation
- Review treatment options and timelines
- Communicate with your office as needed regarding care coordination
